Share of cost is the amount your parent has to put toward their own care each month before Medi-Cal covers the rest. It comes out of monthly income, not out of savings.
People confuse it with the asset limit constantly. They are separate systems, and understanding the difference stops a lot of unnecessary panic.
Assets get you in the door, income sets the monthly bill
Eligibility is about resources: the $130,000 limit for a single applicant as of January 1, 2026, with the house and a vehicle exempt. Share of cost is about income: Social Security, pensions, annuity payments, whatever arrives monthly.
So a parent can qualify for Medi-Cal and still owe a substantial monthly share of cost. That is not a mistake or a denial. It is how the program is designed.
The number that shocks families
For someone in a skilled nursing facility, most of their monthly income typically goes to the facility as share of cost, with a small personal needs allowance retained. If your father has been mailing money to a grandchild every month or covering a car payment, that stops. Plan for it before the first bill arrives rather than after.
What comes off the top
The calculation allows certain deductions before the share of cost is set. Health insurance premiums, including Medicare premiums, are commonly among them. In the married case, an allowance may flow to the at-home spouse so that spouse is not left below a minimum monthly income.
That spousal allowance matters enormously and it is routinely under-claimed. If your mother is at home and your father's pension was the household's main income, this is the provision that keeps her lights on. Ask about it specifically.
“The county is working from what it has on file. If a deduction your parent is entitled to never got documented, nobody calls to point it out. I have seen shares of cost come down meaningfully just by submitting paperwork somebody assumed had already been sent.”
Delia Vasquez-HartGet the documents right
- Social Security award letter showing the current gross benefit
- Pension statements, including any survivor election
- Proof of health insurance premiums paid
- Annuity or structured payment documentation
- For a married couple, the at-home spouse's own income and housing costs
If the number looks wrong
Share of cost determinations can be reviewed, and errors happen, usually because a deduction was not documented or the income figure on file is out of date. If the amount does not match what you calculated, do not simply start paying it and hope. Gather the paperwork and ask for the calculation.
Questions we get asked
Does share of cost come out of my mother's savings?
No. It is calculated from monthly income. Savings are relevant to the asset limit, which is a separate test.
How much can my father keep for himself?
A small personal needs allowance is retained for incidentals. It is genuinely modest, which is why families often continue covering small things like haircuts and phone service.
My mother is at home and my father is in the facility. Does she keep any of his income?
There are provisions intended to prevent the at-home spouse from being left below a minimum monthly income, and an allowance may be directed to her. Raise this specifically, with her income and housing costs documented.
Can share of cost change?
Yes. It moves when income changes, such as a Social Security cost of living adjustment, and it should be updated when deductions change.
