Aid and Attendance is an increase to the VA pension for wartime veterans and their surviving spouses who need help with the ordinary activities of daily living. It is federal, it is not Medi-Cal, and the two programs have entirely separate rules.
The reason it is worth checking: it can help pay for assisted living, which Medi-Cal generally will not.
Where it actually helps
Medi-Cal covers skilled nursing but generally not assisted living or board and care. That gap is where most California families get financially wrecked, because assisted living is what an aging parent usually needs first. Aid and Attendance is one of the few sources of money that can be applied to that gap.
It is not enormous. It will not cover a Westside memory care community by itself. But combined with Social Security and a modest pension it sometimes closes the distance between a facility a family can afford and one they cannot.
Who qualifies, broadly
It generally requires wartime service, a discharge that was not dishonorable, a need for assistance with daily activities or a comparable circumstance, and income and net worth within VA limits. Surviving spouses of qualifying veterans may be eligible in their own right, which is the piece families most often miss entirely.
The paperwork problem
Find the DD-214 before anything else. That discharge document proves the service, and if your father cannot locate his, requesting a copy takes time you would rather spend on the application. Look in the fireproof box, the safe deposit box, and the file cabinet drawer nobody has opened since he retired.
- DD-214 or equivalent discharge documentation
- Marriage certificate, and a death certificate for a surviving spouse claim
- Physician documentation of the need for assistance
- Current income and asset information
- Facility contract and monthly cost statements if care is already in place

There is a cottage industry of advisors who offer free help with Aid and Attendance and then sell the veteran an annuity or an insurance product. Accredited representatives, including accredited attorneys, VA-recognized veterans service organizations, and county veterans service offices, exist and are the appropriate route. County veterans service offices in Los Angeles County help at no cost and sell nothing.

Running both programs
Aid and Attendance and Medi-Cal have separate eligibility rules, and steps that improve one can complicate the other. VA net worth rules and California's reinstated Medi-Cal asset limits are different tests applied to overlapping facts, so a restructuring done purely for one can create a problem in the other. Sequence matters, especially with California transfer treatment still unsettled after January 1, 2026.

Does my father have to have been in combat?
The general requirement is qualifying service during a wartime period, not combat itself. The service dates on the DD-214 are the starting point.
Can Aid and Attendance pay for assisted living?
It is a cash benefit the recipient can apply toward care costs, which is why families use it for assisted living, the very setting Medi-Cal generally does not cover.
My mother is a widow. Does she qualify on her own?
Surviving spouses of qualifying veterans may be eligible. It is the most overlooked path to this benefit, so pull your father's discharge papers even though he has been gone for years.
Will applying to the VA affect Medi-Cal?
They are separate programs with separate rules, but they interact through income and asset treatment. Do not restructure for one without checking the effect on the other.
