What Medi-Cal Actually Covers for Long-Term Care

Medi-Cal pays for skilled nursing facility care. It generally does not pay for assisted living, board and care, or the caregiver you hire to come to the house.

That one distinction decides most of what families want to know, and almost nobody explains it before you have already toured four facilities you cannot afford.

Skilled versus custodial, and why the words matter

Skilled care means care that requires licensed medical personnel. Wound management, IV therapy, rehabilitation after a stroke, monitoring that a nurse has to do. Custodial care means help with the ordinary business of being alive: bathing, dressing, eating, getting to the bathroom, remembering the stove is on.

Custodial care is what most aging parents actually need, and it is what most people mean by long-term care. It is also the kind Medi-Cal is least likely to pay for outside a skilled nursing facility.

The assisted living gap

That pleasant place in Encino with the courtyard and the activities calendar, at $6,800 a month? In most cases Medi-Cal does not pay for it. Families discover this after they have moved a parent in and burned through the savings that were supposed to last five years. Ask about payment sources on the tour, not after the deposit.

What is generally covered

  • Care in a licensed skilled nursing facility, once medical necessity is established
  • Doctor visits, hospital care, and prescriptions under general Medi-Cal coverage
  • Certain home and community based services through waiver programs, which have limited slots and waiting lists
  • Durable medical equipment when prescribed

What is generally not covered

  • Assisted living and board and care, in most circumstances
  • Private caregivers you hire directly
  • A private room, unless medically required
  • Memory care marketed as a residential product rather than skilled nursing

Medicare is not the same thing, and it is not the answer

Medicare covers a limited stretch of skilled nursing after a qualifying hospital stay, and it is genuinely useful for rehabilitation. But it runs out. It was never designed for long-term custodial care, and when families say their parent's care is covered, they usually mean a Medicare rehab benefit that is going to end on a specific date somebody has already written on a form.

Find that date. It is the real deadline in your family's situation, more than any legal deadline I could give you.

Questions we get asked

Can Medi-Cal pay for a caregiver at home?

Sometimes, through home and community based waiver programs, but availability is limited and there are waiting lists. Do not build a plan around it without confirming your parent can actually get a slot.

My father is in rehab now and Medicare is paying. What happens when it stops?

Either he goes home, moves to a lower level of care that somebody pays for privately, or stays in the skilled nursing facility with Medi-Cal picking up the cost if he qualifies. The application takes time, so start before the Medicare benefit ends, not after.

Does Medi-Cal cover memory care?

It depends on how the facility is licensed. Memory care inside a skilled nursing facility is a different question from memory care as a residential assisted living product. Ask the facility what license it holds.

Will they move my mother if she goes on Medi-Cal?

A facility that accepts Medi-Cal and has a certified bed available generally continues her care. A facility that does not participate is a different situation, which is why you ask about Medi-Cal certification before admission.
